A large real-world comparison
In July 2026, researchers published a target trial emulation built from real-world data on 217,920 matched adults, comparing tirzepatide with semaglutide across heart and stroke outcomes (JACC Advances).
They reported that tirzepatide was associated with a lower risk across the studied cardiovascular and cerebrovascular outcomes at both one and three years, with the size of the association differing by diabetes status.
Because this is an observational comparison rather than a randomised trial, it shows an association, not proof of cause. It is a signal from a very large dataset, and researchers themselves frame it that way.

Why the dual mechanism draws research interest
Tirzepatide's pull for researchers is the two receptors at once. Where a single-agonist peptide engages GLP-1 alone, tirzepatide adds GIP, and much of the current research is trying to unpick how that combination behaves compared with single-pathway agents.
